Haemophilus influenzae merupakan bakteri gram negatif berbentuk kokobasil. Bakteri berkapsul ini memiliki sifat non-motil dan anaerob fakultatif. Bakteri ini pertama kali diidentifikasi oleh Pfeiffer pada 1892 yang menduga bahwa bakteri ini adalah penyebab influenza. Penyebaran bakteri ini terjadi melalui tetesan udara maupun kontak dengan sekret. Virulensi bakteri H. influenzae terkait dengan polisakarida yang dimilikinya. Strain H. influenzae yang tidak memiliki kapsul lebih relatif jinak, sedangkan strain yang memiliki kapsul atau lapisan polisakarida dapat menyebabkan penyakit yang lebih serius.




Karakteristik


Haemophilus influenzae memerlukan nutrisi dan lingkungan khusus dalam pertumbuhannya. Bakteri ini tumbuh pada lingkungan dengan suhu 35-37° C dan konsentrasi CO2 5%-10.% Faktor X berupa hemin dan faktor V berupa NAD (Nikotinamida adenina dinukleotida) juga dibutuhkan oleh Haemophilus influenzae dalam pertumbuhannya. Faktor X diperlukan oleh H, influenzae untuk pembentukan enzim yang mengandung heme (sitokrom, katalase, oksidase). Media berbasis agar darah (blood agar) yang berwarna coklat dapat digunakan untuk pertumbuhan bakteri ini, karena mengandung faktor X dan V.




Patogenitas


Haemophilus influenzae merupakan bakteri dengan rentang patogenitas yang luas. H. influenzae tipe b (Hib) diidentifikasi sebagai salah satu penyebab penyakit invasif seperti meningitis pada anak. Bakteri ini juga dapat membentuk kolonisasi yang komensal di saluran pernapasan dan memicu pneumonia. Infeksi pada saluran pernapasan bawah dapat pula disebabkan oleh strain H. influenzae yang tidak berkapsul. Penyakit pernapasan lain seperti penyakit paru obstruktif kronis(PPOK) berupa bronkitis, bronkiektasis, dan fibrosis kistik dapat pula terjadi akibat Haemophilus influenzae. Kemampuan bakteri ini dalam melakukan kolonisasi sebagai patogen berkaitan dengan kurangnya ketergantungan pada mekanisme perlekatan tunggal dan kemampuannya untuk memberi respon cepat terhadap mekanisme pertahanan inang melalui variasi antigen protein dan enzim.




Pencegahan



Pada pasal 6 Permenkes Nomor 12 Tahun 2017 dinyatakan bahwa imunisasi dasar perlu diberikan pada bayi sebelum berusia satu tahun, yang terdiri atas imunisasi terhadap beberapa penyakit, termasuk pneumonia dan meningitis yang disebabkan oleh Haemophilus influenzae tipe b (Hib). Vaksin Hib konjugat merupakan sediaan cari dari polisakarida kapsul poliribosilribitol fosfat (PRP) yang telah dimurnikan dari Hib. Vaksin Hib dapat diperoleh secara komersial dalam bentuk sediaan monovalen dan berupa vaksin kombinasi yang mengandung DTP (Difteri, Pertusis, Tetanus).




Penanganan


Pemberian antibiotik oral seperti β-laktam merupakan terapi lini pertama terhadap pasien H. influenzae.
