Hepatitis (plural: hepatitides) adalah peradangan pada hati karena toksin, seperti kimia atau obat ataupun agen penyebab infeksi.
Hepatitis yang berlangsung kurang dari 6 bulan disebut "hepatitis akut", hepatitis yang berlangsung lebih dari 6 bulan disebut "hepatitis kronis".




Penyebab


Hepatitis biasanya terjadi karena virus, terutama satu dari keenam virus hepatitis berikut: A, B, C, D, E , atau F. Hepatitis juga bisa terjadi karena infeksi virus lainnya, seperti mononukleosis infeksiosa, demam kuning, dan infeksi sitomegalovirus. Penyebab hepatitis non-virus utamanya alkohol dan obat-obatan.




Jenis virus hepatitis


Virus hepatitis A
Virus hepatitis A terutama menyebar melalui vecal oral. Penyebaran ini terjadi akibat buruknya tingkat kebersihan. Di negara-negara berkembang sering terjadi wabah yang penyebarannya terjadi melalui air dan makanan.

Virus hepatitis B
Penularannya tidak semudah virus hepatitis A. Virus hepatitis B ditularkan melalui darah atau produk darah.
Penularan biasanya terjadi di antara para pemakai obat yang menggunakan jarum suntik bersama-sama, atau di antara mitra seksual (baik heteroseksual maupun pria homoseksual).
Ibu hamil yang terinfeksi oleh hepatitis B bisa menularkan virus kepada bayi selama proses persalinan. Hepatitis B bisa ditularkan oleh orang sehat yang membawa virus hepatitis B. Di daerah Timur Jauh dan Afrika, beberapa kasus hepatitis B berkembang menjadi hepatitis menahun, sirosis dan kanker hati.

Virus hepatitis C
Menyebabkan minimal 80% kasus hepatitis akibat transfusi darah.
Virus hepatitis C ini paling sering ditularkan melalui pemakai obat yang menggunakan jarum bersama-sama. Jarang terjadi penularan melalui hubungan seksual. Untuk alasan yang masih belum jelas, penderita "penyakit hati alkoholik" sering kali menderita hepatitis C.

Virus hepatitis D
Hanya terjadi sebagai rekan-infeksi dari virus hepatitis B dan virus hepatitis D ini menyebabkan infeksi hepatitis B menjadi lebih berat. Yang memiliki risiko tinggi terhadap virus ini adalah pecandu obat.

Virus hepatitis E
Virus hepatitis E kadang menyebabkan wabah yang menyerupai hepatitis A, yang hanya terjadi di negara-negara terbelakang.

Virus hepatitis F
Jenis baru dari virus hepatitis yang telah terdeteksi baru-baru ini. namun belum terlalu diketahui.

Hepatitis A biasanya akan sembuh dengan sendirinya tanpa menjadi kronis. Setelah sembuh, maka akan kebal terhadap Hepatitis A, tetapi tidak kebal terhadap jenis penyakit hepatitis yang lain.5 persen dari penderita Hepatitis B akan menjadi kronis, karena tidak ditangani dengan baik.Pada pemakai narkoba suntikan yang menggunakan jarum bersama-sama yang marak pada masa lampau, maka 18 persen tertular Hepatitis B, 40 persen tertular HIV dan 70 persen tertular Hepatitis C. Jadi Hepatitis C sangat mudah menular melalui transfer cairan (virulen).Penderita Hepatitis C sebenarnya hanya 0,8 persen, tetapi sebagian besar akan menjadi kronis, sehingga jumlah penderita kronisnya hampir sama dengan penderita Hepatitis B kronis, yaitu sekitar 1 juta orang.




Pencegahan

Vaksin tersedia untuk pencegahan hepatitis A dan B yang merupakan vaksin tunggal ataupun vaksin gabungan. Kekebalan terhadap Hepatitis A mencapai 99-100% sebulan setelah menerima vaksin yang ke-2 kalinya (vaksin yang kedua 6 bulan kemudian setelah yang pertama). Vaksin hepatitis A tidak boleh digunakan untuk yang berusia di bawah satu tahun. Vaksin Hepatitis B telah tersedia sejak tahun 1986 dan telah diterapkan sedikitnya pada 177 program nasional imunisasi untuk anak-anak. Kekebalan terjadi pada lebih 95% anak-anak dan dewasa muda yang menerima 3 dosis rekombinan vaksin, sebulan setelah vaksin yang ketiga (jadwal vaksinasi adalah 0, 1 bulan dan 6 bulan). Vaksinasi pada bayi yang berumur kurang dari 24 jam dapat mencegah penularan penyakit hepatitis B dari ibunya. Organisasi Kesehatan Dunia (World Health Organization) merekomendasikan vaksinasi pada semua anak, terutama yang baru lahir di negara-negara dimana hepatitis B marak terjadi (seperti Indonesia, terutama di NTB dan NTT) untuk mencegah penularan secara vertikal dari ibu ke anak.
